Please read the following statements carefully. They explains what you can expect from Beholding & Becoming Counseling and what will be expected from you. You will be required to acknowledge and consent to these policies when completing your intake form.
Statement of Confidentiality | Counseling will adhere to very strict confidentiality standards. Client information is managed using procedures designed to protect the privacy and security of personal data. Counseling records are strictly confidential. In order to protect your right to confidentiality, your written authorization is required if you desire that information be shared by us about your counseling to another person or agency. In the case of marriage or family counseling, there is limited confidentiality, meaning that confidentiality belongs to the relationship and not to the individual. Although we make every effort to maintain confidentiality, some situations require us to share information with others. For example, the following situations may require disclosure to governmental authorities or affected persons/entities: disclosure of child/dependent/spousal abuse, disclosure that you present a risk of harm to yourself and/or others, or a disclosure required by subpoena or other court order from a federal, state or local agency or court.
Hold Harmless Agreement | In consideration of the offer and provision of biblical counseling, you agree to release, waive, discharge and covenant not to sue Beholding & Becoming Counseling and/or its affiliates, its officers, servants, agents and employees, including individual counselors, as well as GraceLife Church and/or their affiliates, officers, servants, agents and employees (hereafter referred to as "releasees") from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or relating to any loss, damage or injury, that may be sustained to you, or to any property belonging to you relating to your participation in biblical counseling with Beholding & Becoming Counseling.
